    Panov Attack in the Caro-Kan leads to such a position.

    Panov Attack vs. Caro-Kann, Tarrasch Defence vs. Queen' Gambit, Tarrasch vs. French...various others I can't remember off the top of my head.

    some lines in the Spanish.

    As you can surmise, studying IQP positions can may off handsomely.

    An outstanding book for that is Alexander Baburin's "Winning Pawn Structures" which covers not only IQP, but also related families such as pawn couples and hanging pawns in both endgame and middlegame settings.

    I don't think it is. In the QGA, Black would have played c7-c5 in one go, so White would have to have lost a tempo somewhere as well. And Black doesn't usually go Nbd7 in the QGA.

    What would the QGA move order have been? Just that Black plays ...dxc4 at some point (as he so often does in the Slav and Semi-Slav) doesn't make it a QGA.

    If White wants an IQP, he can get one in the Colle-Koltanowski. Palliser discusses this option in his book on that opening.
