I have searched but have had no success in finding a source that explains why/how an opening has its name, i.e. Sicilian, Nimzo Indian, Caro-Kahn, etc. Is there a book or site that might explain this? I know some names are obvious but what about the many that are not?

MCO introductions usually include (or used to anyway) how openings got their names. It's a good idea for a site though.
