11... e5 strikes me as your opponent's biggest blunder, followed closely by 13... exd4.

I agree that 11...e5 was unnecessary, but that was a very well conceived and executed attack. 13...exd4?? was completely losing for black. Very nice!
I always thought that ... d5 was the thematic move in this defense. The idea of accelrating the fianchetto is to get to d5 in one move instead of two. I don't have my ECO with me and I don' know these lines well, but it looks like your opponent transposed back into a standard Yugoslav.
Yes -- saving a move with d7-d5 instead of d7-d6-d5 is the idea behind the accelerated dragon and it's also supposed to preclude the Yugoslav attack, but in this case it does neither.
