Until this game proceeded to the 14 moves, the black didn't develop any pieces except one black knight.
I think it is the reason why the black lost this game.

Instead of the black move 13...a5, the black queen had to move to Qa5 and empty the d8 square so that the black king can avoid checkmate.

The reason you can rarely finish your arrowhead opening (I think you mean 'formation') is because it's terrible.  If Black had used the tempi you were wasting on pawn moves to develop pieces, he would have had counterplay.  You guys both played with no development (Black had 3 piece moves in 14 moves! Yikes!) and that led to a quick game.
