12. ... b6 lost you a pawn.

 

13. ... Nc6 lost you material because after 14. Qb5 white is attacking your undefended knight and your rook with a bishop.

White should win easily with the simple 40. Rf2 stopping the checkmating threat.

White should've traded Qs at move 17 with an easy win a R&P up.

If white is rushing to get checkmate, 37. Qf8 threatening Rf7 would be the way to go. If 37. ... Rg7 then 38. Rf6 threatening Rh6 and if 37. ... Rxg5 38. Rf7+ Kg6 39. Qg7#.
