I don't know If you know this already or not, but Move 27 lets him play Qxf3 and when you take back with the rook with Rxf3 he plays Bxf3, winning material in the process.

You keep reporting what the computer thinks, but what do you think? Do you understand why the computer scores -5.03 after 27.Qf3??? Do you understand why the computer thinks a good move was missed when Black played 27...Qb6??  These aren't tricky chess questions; all you need to do is look at the board and count! 27.Qf3?? is a terrible move because after 27...Qxf3 28.Rxb3 Bxf3 you've lost a rook, and since the rook on d1 and the knight on f5 are under attack, you're about to lose more material. 27...Qb6?? Is a terrible move because black refuses to take the material you're offered him. 

There is one thing above all that you need to work on, and that is actually looking at the board and noticing what is being attacked and how it's being defended. And you need to do this on every turn. Until you look carefully at the board and count defenders and attackers, you will not improve.
