How did I let this game slip away?

I haven't played chess for a long time and made a big effort to try to win this one but from being three pawns up at one stage I let it slip away. I feel like I am particularly weak during the end game.  Any advice would be appreciated.

You had a lot of opportunities to mate him. The biggest one that you should have seen was 31. Qf5#. You should probably try working on finding checkmates.

27 Qh7+ mates in a couple moves (also on move 28).

 

30 d5+ appears to win.  30... cd 31 Rb6+ Qxb6 (31... Bc6 32 Rxc6+ Kd7 33 Qg6) 32 Qh6+ Ke7 33 Nxd5 mate.  Or 30... Kd6 31 Qh6+ Kc5 (31... Ke7 32 d6 mate) 32 Qe3+ Kd6 33 Rf6+ Ke7 34 Qxe5 mate. 

 

34 fe/Q+ wins a piece. 

 

I think earlier you should've unpinned your knight as quickly as possible (say with 25 Kf1).  The sooner it could jump into the fray, the better.
