How do you avoid blunders? I'm not talking about long-hard-to-see-sacrificial-combinations. How do you avoid simple oversights? If it came with experience then how come even grandmasters do it sometimes? Is there some formula or way of thinking that will help?

I stop and ask myself what could go wrong if I make this move.... And I try to see what could go wrong a couple moves in. The more i play the less i blunder because i think im able to read the board better. But I still blunder, alot though. Also i would reccommend playing alot of long games live until your making few blunders and then move to blitz. Ive noticed after playing blitz helps me avoid blunders in long games.
