Another one is Starting Out: 1 d4! by John Cox. Cox's choice in the QGD is the Exchange Variation (if that matters to you.) It's a repertoire book, so it covers Indian defenses, Dutch, Modern, QGA, Slav, etc. Coverage is pretty thin in some areas, but that's to be expected with most repertoire books.

Are there any good books that go into the queens gambit and then followed by NC3 and other variations not just when black responds with d5?
