How to play against the Sicilian?

Blast the Sicilian!!!  I really struggle in these positions.  Most of the time against better players they end up in a better endgame where I either have to hope for a draw or have to fend off the attack, then hope for a draw, or just die. 

How can I make the positions complicated enough to hopefully play for a win against the sicilian?  Is it even possible?
