I think there was one in the "Game of the Century" between Byrne and a young Fischer in a simul. In any case yeah, this was more a royal fork than anything else.
What is a windmill?
it is the destruction a Knight causes when constantly checking the King by a series of 'discovered checks' (when moving the Knight places the King in check by another piece). just check the game out to see it in action.
I have one "tactical idea" I saw only once, and only in that game. Maybe not that interesting a move but still quite rare I guess. I had black, of course in a random internet game, white is some Romanian 2500+ IM (if memory serves right... yes it serves right):

normally when i lose to a patzer it's quite like that.
