Survey changes:
Added the button on the main page
Fixed a couple questions to be checkboxes instead of multiple choice
Added a few new checkbox questions, with text fields to list items not in the checkbox questions

Maybe along with the rating comparison, there could be an anonymous list of each survey taker and their ratings.
