Either way.
The KIA is sometimes a bit boring if Black responds to e4 with ... e5, so one possibility is to open with 1. e4 and play the Ruy Lopez (or whatever you like) if Black responds with ... e5 (thus avoiding a KIA vs e5) while against the Sicilian, or the French, or the Caro-Kann, or the Alekhine, you play 2. d3 and 3. Nbd2 then head into a KIA.
