Thanks for resurrecting this thread. 4...Nd4 is not so new. Here are a couple of Weaver Adams games from 1948 featuring this move. I'm not sure which one was played first.
I got the Milton Kagan game from the book How to Play Chess.
And inside the book was a clipping from the Boston Herald, unfortunately I don't know the exact issue.
