The positions after Black's 34th, 36th and 38th moves are identical.
You blew it.
The position did repeat 3 times. Start at the end of the game and work backwards. I noticed the black king on b7, a black rook on c8, and 2 white rooks on d5 and h5, 3 separate times.
You should have traded the last rook and then make a queen from your numerous pawns. The game would have been easier to win that way.
Draw by repetition happens like that sometimes. The three fold repetition doesn't have to follow each other.
