They're not. Accounts marked as "closed: inactive" means that they're closed without being banned; usually that means someone decided to close their own account.

Indeed. That is the only way you can get that particular symbol.

I've also seen that symbol in cases where staff closed an account but it wasn't a ban, such as when someone asked staff to close their account. But self-closed accounts are the main explanation.
