if youre too scared to leave the house then there are always fide arena titles that are earned online. But other than that i genuinely dont think there is any other way you'd manage to get a proper title if you cant leave your house. If it makes you feel better if youre in a playing hall no one speaks and you can get out of your chair to walk around or even leave. But if you really want a title you should work up to that level first before you even have to worry about that because its a long hard road and if you arent a kid then its gonna take you a long time or maybe never.

The thing is there are probably very few people like you that are interested in chess and want to compete and they cant really make special exceptions just for you, it would cost them too much, for an official game they need an arbiter and you need an opponent so if they made special exceptions for you that would mean they transport the opponent and arbiter to your house just for you to play a game and you arent an important player so unfortunately the chess scene moves on with or without you. Hopefully you can get over your fear and or join a club where it will only be a 4 v 4 match against another club and maybe that is better for you but yeah sorry for being so direct with it but its the truth good luck

*Snip* I've read previously that it's only possible to acquire a title of any meaning by playing OTB. I have a health condition called agoraphobia, *snip* For those that will say "A title really doesn't mean anything", I would just say that those small achievements matter to someone like me *snip*

Each one of us has to find meaning in our lives. Many of us chase after places of "value" derived from social norms but these almost never make anyone happy.
How about creative pursuits like publishing?
First, these days you can get a sense of accomplishment from your online chess results. The gatekeepers will keep telling you that OTB is all that matters, but if you manage to reach, say, 2900 on chess.com (I am not saying it's going to happen, but suppose it will), even if you have no official FIDE rating, people will definitely pay attention to you. If you can get really good online, it will be noticed, don't worry.
Other than that, I strongly recommend talking to your mental health counselor regarding things blocking you from participating in OTB chess events rather than seeking advise here.

I have a health condition called agoraphobia, and even though I really suck at the game currently, it would be a goal to eventually hold some title. Unfortunately, this condition limits my ability to ever leave the house. One might say that the love of the game is what's the most important, and despite how much I suck, I do love the game. Even losing is fun. And it would just give me some inkling of hope that life isn't so bad if there was an exception if I studied, practiced, and became good enough to compete. There's not much I can achieve being locked in my own personal prison, so I just wanted to reach out and see if there's any hope at all, and worth putting in the practice and study. For those that will say "A title really doesn't mean anything", I would just say that those small achievements matter to someone like me who can't really do much besides watch the world pass them by. So it would at least give me a goal to strive for, and maybe provide a little encouragement. Thank you for anyone that provides at least some semblance of empathy and maybe there's a solution I'm not aware of.
