I think it would be nice if chess.com provided the game as well as the username along with the notification rather just giving away points.

Luckily I do not encounter many cheaters anymore, but one thing I did was to enter games that I thought were against cheaters in my library. That way, I could filter back a month or two after reporting, and check my accuracy vs. chess.com's cheat detection system.
They only refund points for games played in the last 40 days (IIRC), so it would be a simple matter of looking back at your game history and seeing who was banned.
And to answer your other question, when that happens for fair play reasons, the other account is permanently banned. The person can create a new account, but not use that one again (there is an appeals process, but they almost never overturn their initial ruling).
Idk but some tournaments offer membership rewards if you win them, so maybe they're trying to win something in particular and then just .. Stop cheating or something? Idk just a thought.

I could swear that it was last 90 days (3 months) or last 100 games, whichever comes first so to speak.

You might be right. But it is time boxed and the time frame is not all that large.

Yeah, that is for sure. For instance one person can play against you and then play 100 bullet games in a few days, and you get nothing. At least it was like that 2 or so years ago.
