It was the fact that he doubled blacks pawns on the e file and managed to get away with it after blundering away the position with Nd2. There's really no way that Sergey did not see the Nf2 move. How could he?? There's a queen and rook battery hammering down on it. No GM overlooks something like that.

 

This point is actually proved a move later when Karjakin again ignores the capture after Magnus played Qh5. Impossible for him to not see the capture twice. He avoided the move intentionally and I could care less what Sergey said at the press conference. He knew damn well what he was doing.

They followed an engine v. engine game up to move 13, when Carlsen deviated.   In engine tournaments, Carlsen's opening choice performed pretty well. I wonder if Carlsen reviewed engine tournament games in his preparation.  

Up to move 9 Carlsen had played the black side against Shirov in 2011.  At move 10, they leave "human territory" with the last major OTB game played in 2011 between Fedorchuk and Delorme.  But it is not until move 13 that they leave engine v. engine territory.
