How can I maintain focus an entire game?

A lot of times I will play 10 or 15 or 20 sometimes even 40 moves in a row that are either grandmaster level moves, moves a GM would consider a candidate move before they think of a better one, or slight mistakes, and minor innacuracies. Basically "Good play"...
Then I go and do something stupid like make an unsound sacrafice, or flat out drop a piece on negligance, or miss a perpetual check. I take a game where I was winning, or equal or just a little worse and turn it into a lost game in 1 move.
