How do you come up with plan in MG?

Often I play the opening ok.. it is the Middle game i have problem with. Most of the time I have no idea what to do next. I just start reacting to moves of my opponent and become complete defensive. How can I help myself?

Look for the weakness in your opponent's pawn structure and attack it.
