How do you improve your game (Opening, Tactics, Endgames)?

I feel I have hit a plateau and can not improve further. 

Opening: I've got enough games under my belt to not get Fool's Mated or Scholar's Mated, but all I do is muscle memory the London or Vienna as white or just mirror the pawn as black (1.e4 e5 or 1. d4 d5). Whenever I try to recall a specific sequence beyond those first several moves I freeze up and end up putting myself in an unfavorable situation. I want to be able to go a little more deeper in to established openings and not just the first 3 moves or so. 

Tactics: I'm making less obvious blunders than before and discovering forks and skewers. But again, all I'm finding are my next moves or the move after IF my opponent does not discover my intentions (which they often do). How do I get to the point where I can go 3 or 4 moves deep and consider multiple possibilities in a game?

Endgames: I can recognize things like back rank mates, smothered mates and perform the Queen and King mate without messing up. The puzzles have definitely helped but I do want to be able to recognize less obvious patterns and recognize them faster.

"Whenever I try to recall" ++ Do not try to recall, think.

++ That will not help you. Just play and analyse your lost games to learn from your mistakes.
