Make it easier.  Include credit card numbers - social security/government ID numbers -
include copy of driver's license and passport.
Get it over with.

If you want to be even more thorough
include copy of birth certificate - job contact info - home address.
Don't forget to include photos and names of your kids - and where they go to school.
Oh yeah - almost forgot !  car description and license plate number.
Be thorough!

Include email address so they can email you and then by emailing back they can confirm your location.
