So in the Italian game and from many guides on the good place for a bishop in the opening, one is placed at c4. But I feel uncomfortable having it there, as it is not backed up by pawns and not easily by knights. often I have to bring it to d3 or b3. so what am I missing?

You are missing nothing. The bishop will get back to b3, or be supported by something whenever needed.

nothings attacking it. So now I guess it is to be used later, no immediate plans in most lines..
