when should you swap a queens?

Basically the queens represent dynamic play which can compensate for static disadvantages (like poor pawn structure). You trade them when you want to highlight your opponent's static disadvantages, or trade them when you want to diminish the opponent's dynamic advantages (like if your king is vulnerable and they are attacking).

Or sometimes, much more simply, their queen is on a very good square, and there is no safe or active square for your queen. Their queen is better than your queen, so you would trade in that case purely for the sake of piece activity.

A good piece is generally:

 - centralized or near a weak enemy king
 - influences many squares (not blocked by other pieces)
 - Is safe (can't easily be chased away by enemy pieces)
 - Is protected (isn't vulnerable to tactics like forks)

When it is to your advantage to do so.

If that answer isn't particularly helpful, that's because the question is vague.  Without specific positions to consider, there can be many arguments both for and against the trade.

And that's not even considering the possible psychological reasons to do so - some people just don't know how to handle a queenless middlegame, so trading queens might be a good idea even if it isn't objectively the best move.
