Underpromotion troll possible?

Can someone come up with a position, where undepromotion would be the only move? The context is playing against a low rated player - someone that doesnt know undepromotion exists - and get them into a position, where they think they got mated (or stalemated), but in fact they have a move, which is underpromotion to either rook, bishop or knight, so they burn all their remaining time desperately looking for a move, and lose on time wondering why loss or draw wasnt announced. Its even better if the resulting position was a win for the beginner. Lets see what we can come up with!

That's a famous position. Anybody know where it was first published?

Was that problem from Fine's Basic Chess Endings?

The position arises from a problem known as the Saavedra position. It is much older than Fine’s book.
