You should post the game score.
Draws can happen without your agreement if a position is repeated three times and your opponent makes the claim for a draw. They can also happen if the game has gone 50 moves without a pawn move or capture. Again, a claim has to be made. (On chess.com, pushing the "draw" button in these situations is the way to claim.)

You're right. Strangely enough, you had another draw by agreement with the same player when he was clearly winning:

The other draw against that player was agreed by both of us to "even up" what had happened here. He seemed confused about it, too. The second draw was definitely consensual.

Playing on my android chess app, I was having a good game and it randomly just drew the game. I never asked for a draw and never accepted an offer. Bit odd?
