I just lost a 41-move game due to time out. On move 40, I moved (white) and had 1:15 on the clock. My opponent moved with 3:15 remaining. After move 41, I had 1:13 left. When my opponent moved, my clock immediately expired. What happened?

Sounds like you got disconnected from the live server.
