However, increments alter these categories somewhat, I believe, and I haven't quite been able to understand exactly how they affect them. For example, I generally play 5 min. games with a 10-sec. increment. That is still considered blitz in my stats. If I were to play 5 min. games with a 15-sec. increment, would that become standard time controls? How are the cut-off points determined?

The cut-offs are based on the game being 40 moves (as an average) and the initail time plus the increment being added together...
i.e:
A time control of 2 | 12 therefore results in a Blitz (not Bullet) game like so:
2 minutes + (12 seconds x 40 moves = 480 seconds) = 10 minutes
Since there are dozens of possible time controls when you throw in increments it would take a while to find all the cut-offs for each of the rating catagories...in your two cases 5/10 is blitz (10 x 40 = 400 seconds) plus the intial 5 min = 11 min 40s...however a 5/15 is a standard game (15 x 40 = 600 seconds) plus the initial time of 5 min = 15 min
