The name I found associated with this is the Hodgson attack. Probably still Julian Hodgson that made the Trompowsky famous (1. d4 Nf5 2. Bg5).
Julian is a present day master, so this opening probably has several names. Levotsky is probably one of them!
Background: while playing OTB, I had a bad tendency of playing QGA as black. One time, I decided that I would play QGD. Unforuntately, my opponent didn't cooperate and played 2. Bg5. I have never seen this move before. I played h6 and got clobbered. Steve and I decided to play this opening line and see where it goes. Here is our latest game:
