I think that the h5 pawn sac is a quite normal theme when the opponent short castles isn't it?
I don't think that 14... Bxh6 is good. I think I would of went 14... Be6 instead. I wouldn't want to just give the white queen h6 like that. If I had to put my finger on it, I would say that this is where black messed up. It is just a guess though... These are just my thoughts.
