It essentially works the same as OTB. Black starts the clock, then white moves, so the clock is running when the move is made.
First move takes time
At least on Lichess there's a delay before the clock starts running for the 1st move for both sides, so it's something that has been tried for online chess before and seems to not be such a bad suggestion. It's helpful especially for bullet and/or 960 games imho.

That's not the way timed chess should work.
The thing is:
It literally is counted as a premove for me. I just rattle out e4 in maybe 0.5 seconds AT LEAST, and it's been counted as 0.1. I might be able to recall a couple black games where I react to my opponent's first move, whether by caro or by dutch/indian, and it's still counted as 0.1.
In fact, I think the full games themselves take too long. I've queued up 30-second games before, record them, and the games themselves are like 1 minute 20 seconds long in reality due to some lag, intentional delay, idk.
What's the point?
