"Phantom Notifications"

Recently, Whenever we click on a notification, even without another comment we get another notification, It has been deemed as "Phantom Notifs". Does any staff know the cause of this and how to fix it? 

For example, if I get a cc beta note notif, then I click on it. After I click on it, after a couple minutes it gives me a notification for cc beta again even without a new comment.

i also get for like when i load a new page or refresh but then before it is usable it is gone

This has been happening to me for a while now..

same, its annoying

yes it is very anoyying because you have 3 notifs for example and you refresh for any or no reason and then you see 7 notifs and go like "I need to look at these" but then before the site is usable it goes back to three or by chance if you got any notif in that refresh it will add that as well

and one more thing i noticed is that the phantom notif that appers are always +4 than the original

i thorught it is a detail to mention to maybe get the solution
