I'm no chess teacher, but I can tell you what any teacher would. Play less blitz; all you're doing is perfecting your mistakes. Good players love playing blitz and bullet because they have already internalized thousands of patterns that they can recognize and react to immediately. You're not there yet so play more long time controls until you are.

And take advantage of your diamond membership. ChessMentor has all kinds of ways to improve but the most bang for the buck would probably be to hit the tactics courses hard. Keep reviewing until the patterns of typical tactical motifs become internalized. GM Patrick Wolff has at least a dozen courses and there are others. Good luck!
