this looks like something has gone wrong with white's attack

Play Nc6 and g6.

on move three why didn't white take the free e-pawn? no wonder why something went wrong with his attack.

No idea, but i'm seeing a lot of 2.Qh5 now and getting tired of playing Nc6, g6 against it. So i went with d4 to stop the bishop. But looking at the analysis by the engine, i'm just down a pawn with no follow up moves after exd5 and g6 is literally the best move to follow up/develop the rest of my pieces.

sndeww

yes. Most openings don't have a "real" refutation. It's just whether or not black can get a nice edge.

I recommend 2...Nc6 defending the e5 pawn and after 3.Bc4 Nf6!! gaining a tempo on the queen.
