I faced 1h4 2h5 3h6 once. I played 3..nxh6 and i guess he was surprised, with a pause.

First, the Damiano is not refuted. It is a playable gambit, as long as you don't play the insane 1. e4 e5 2. Nf3 f6 3. Nxe5 fxe5?? blunder.

Second, any opening that is recognized as an opening is probably at least half-decent. "The worst" is probably some non-opening like when the guy pushes all 8 pawns in the fist 8 moves.
