How do you get a CM title?

I've recently read the FIDE Title Regulations effective from 1 July 2017 and had some questions: do I need to reach 2000 Elo rating in all categories (Std., Rapid, Blitz)? Do I need norms? If yes, how do you score them?

Read them again. Titles are for over the board games. Games here don't count.
