How does chess.com know if player are computer assited or not?

I started on chess.com 13 years ago, it has changed a lot. Now the higher your ELO the higher the computer-assited cheaters since it is so simple to cheat. Losing against a cheater is frustrating, but how to be sure?  

Question:

1) How can chess.com know when a player is computer- assited?

2) He is systematically banned or warned? 

3) what is the percenter of computer-assited players? 20%, more, less?

The perception of cheating levels is orders of magnitude more than the actual amount.
