Attack and Defence

Whenever a player is in defensive mode and is thinking of protecting himself from checkmate or saving his pieces, he just forgets the sense of attack, the attacking opponent might seem to attack you but it also might be a blunder, but you don't notice this while you are just thinking of defence.

The same applies to Attack, the player forgets that in the greed of domination, he might actually be blundering.

Here is an example, I tried the tactic of pinning my opponent's rook and knight but I did a blunder as my move Qc7 would allow his knight to fork my King and Queen. But as he was just thinking of defending his pieces, he forgot to attack my blunder.
