What time control?

The middlegame isn't your problem.  You're not even getting out of the opening.

You don't follow opening principles.

You miss simple tactics.

You hang material.

it's where I feel I struggle most but lately doing the tactics and lessons is helping me alot, it may be worth doing that?

When you feel you're at a loss for moves, and your position is solid, don't be afraid to repeat a few moves, shuffle about a bit. Most of the time your opponent will start taking risks first, and you just need to finish him while keeping an eye on the clock.
