I think the problem with this game is quite obvious: he brought his King into the action and you did not. Ignoring all the random blunders throughout the game, you did manage to reach a winning position at move 35. Then he begins to bring his King over, and instead of cutting it off, you just let it walk right in with 36.Rh2? I am not quite sure what you were thinking with 36.Rh2, but it seems like you were thinking nothing more than 'Look! an unguarded pawn, I am going to try to take it.' Instead, try to think of the whole endgame as an entity and try to see what strategic points you are going to have to accomplish to win it. So, at move 36, you should not be thinking about how to 'solve' the problem of his kingside pawn majority by immediately trying to get rid of it. As you can see, you were not capable. Instead, think about how to control and contain his pieces. How could black really shove his pawns through? Your pawn is currently sitting on a dark square, so the bishop is a non-entity. ...g4 simply freezes the pawns. ...h4 gets the pawn through after the forced exchange, but if the King is not let into the hunt, then the pawn will never get past h2. So, imagine if you had played 36.Kd2. Look at the squares the black King is allowed. He can walk along the fourth rank or along the e-file, but either way, he is getting further from the action! So now, the kingside has been taken care of, and the focus can shift to the queenside, where you will clearly dominate.
Sit back for a second and take a look at the position after you win the knight. His forces are clearly not suited to take on the position you have and force a win through. So, there is no need to panic and try to snatch pawns, forcing the position to be something it's not. Just accept position you have, control him with your superior force, and slowly walk over him.
