Was lucky to win after a poorly played middlegame

Although I won, I was dissatisfied with the way I played. I think I got out of the opening okay, but my poor play in the middlegame allowed my opponent to gain a significant positional advantage. Then I hung a pawn, and lost two more pawns. At the end I was hoping for a perpetual check, but my opponent blundered and allowed a mate. This was a 30+0 game. I have not done the computer analysis.
