Well, for starters, h6 and a6 were'nt exactly the best moves as they did not open up your peces nor did they help you gain your tempo. Perhaps next time you should tempt white to trade so you can activate your pieces more efficiently. You should have really captured whites last pawn with your bishop, ensuring he cannot make a queen and checkmate you in while.
