It also seems that Death personified in earlier times, such as in ancient Greece and Rome, was in the form of a god (who in turn was given a human form), but it doesn't seem that Death took the personified form of a skeleton (or skull) until the Middle Ages.  Does that seem right?

Yes, they were everywhere after the last great plagues of the 15th century.

I wouldn't know about Ancient Greece personifications of Death. But in those pre-Renaissance engravings, and in books of the Renaissance all the way to the end of the 18th century (mostly by then in Germany), Death took the form of a skeleton, sometimes wearing a black cape, sometimes hiding under rich cloths but showing its skull, sometimes dressed as a woman. Engravings became more and more sophisticated, based on the very rare and beautiful set of copper engravings by Holbein in the 1520's. They had practically disappeared at the time of the French Revolution.

Earlier I'd been looking at "mortis ingrata musica" by Jacob Gole from the early 18th century. I just looked at Hans Holbein's "Danse Macabre" and could see similarities. Gole's work may be even more sophisticated.
