Yeah, I've had a few. Opponent won't draw and tries to avoid repetition only to lose later.
Decline the draw then get mated

It is very common. There is nothing shameful or great about it. The result of the game doesn't change the validity of the offer or the rejection.

I've declined offers of draws before where I felt they were reasonable and that there was a risk of being mated if I continued - generally only if I thought that the position was interesting enough that it was worth risking a loss for.
When I lose that way it doesn't bother me.

Have you ever declined a draw then been mated? I have. Have you been on the other side - offer a draw and then mate the guy after he declines? Pretty shameful/great experience, depending on which side you're on.
