It's pretty common in the Advance French defence structure :
Greco Gambit or Classic Bishop Sac on h7

In last months USCF magazine, they show this gambit or sac play. Seems it always comes from white playing d4 first. Does it ever come from playing e4 first? (or would it be extremely rare?)
