This happened to me the other day but I don't think I've ever seen it before. Is this a rare thing to happen?

Way back in my very low Elo days, I won a game with a pitiful 42% accuracy, while my opponent had 54%. He was suddenly afflicted with what might be the worst case of tunnel vision I've ever seen while up a whopping fourteen points of material (give or take) and hung mate in 1.

I also had a game where, if I recall correctly, my opponent got severely complacent while up nearly 20 points of material and forcibly hung his queen (i.e. my only legal move was to capture it for absolutely free) and then proceeded to completely throw the game, hanging several more pieces for absolutely no reason; it culminated with him pretty much allowing me to promote a pawn and checkmate him, even though he probably had at least seven different ways to stop it.

I was Black in that game; again, if I recall correctly, the evaluation of the position went from something like +M4 to 0-1 in less than twenty moves. I think I got something like 55% accuracy while he had 57%.

That's pretty much how it works. If you absolutely dominate the game but then hang mate or otherwise make some exceptional blunder that makes your position instantly go from completely winning to dead lost, you'll likely have a higher accuracy even though you didn't win.
