Well, sounds like your opening theory is great and your tactics are flawless.
The only thing left is to play in an OTB tournament and after getting beat by a few 7 year olds with 3 digit ratings you'll come back and make a more realistic topic.

Hi, I’m rated around 1200 irl (don’t look at my chess.com ratings, I mostly play elsewhere) and I don’t know how to improve. Here is where I am currently:
Club Play- I am at the top of my small-town chess club and am higher rated than my coach, who is the only person close to my rating.
Opening Play- I have studied Openings the most, as Opening Theory is my favorite chess topic and I often find myself coming out of the Opening with a winning position against WAY higher rated players, sometimes even Class A.
Middlegames- I solve the max amount of tactics puzzles on chess.com and then do some more on elsewhere, and rarely miss tactics in my games. Positionally, I’m studying free material online and I am looking for recommendations for a positional chess book.
Endgames: In the endgames I sometimes find myself lacking or missing faster checkmates than the ones I usually manage to pull off, but I have the Complete Endgame Course by Silman and have read more than the recommended amount of theory.
If anyone has a recommendation, it would be greatly appreciated.
