I'm a long way from it, but how would one be titled?

By beating titled players in FIDE or Nationally rated games.

2200 FIDE rating+apply for the title

I think you can be an Expert or a Candidate Master at under 2200 in the USCF. I'm not sure about FIDE, or about countries other than the US.

So there is FIDE (international - you probably have to play games in Europe but I am not sure), and then there is your country's organization (which would require less travel to obtain).

You are able to play FIDE rated games in any country, but they are just most widespread in Europe.
