How is "accuracy" calculated?

Why sometimes the winner has lower accuracy than the loser, even if the winner has the advantage for the whole game?

They have never really revealed how accuracy is calculated.

It's not that reliable. For example if someone makes one big blunder and the winning person drags the game out, the guy losing can get a very high accuracy by making loads of "best moves" which do nothing at all to remedy the game. For example shuffling a king back and forth.
